Our 24 hour approach is delivered through joint working; training the client, family and those working with them, to create an environment that provides a synergy of skills and opportunities for function and independence.

Who?

We work with clients following injury and /or illness that affect their ability to participate in daily life.
Clients may have obvious severe and complex needs or more subtle needs that may affect their confidence to return to work or leisure activities.

We also offer a bilingual service to Welsh clients.

When?

Services can commence immediately following injury / diagnosis or at any time after this. In our experience it is never too late to explore possibilities for change and improvement in quality of life.
Following a referral a member of the team will contact you discuss your individual needs and arrange an initial assessment usually within 14 days of referral.

Where?

Services are provided in the most suitable environment for clients. This can be their home, work, community, nursing home, clinic or a combination of these.

Integrate Therapy covers North Wales, North West of England and Midlands. Clients can also be seen in other geographical areas by arrangement.

Why?

Integrate Therapy Ltd is a unique service, offering a one stop shop for all your therapy needs.
The team can be used flexibly to meet your needs and are more than happy to work alongside existing Therapists in a case.

You can either use the team as a whole, or pick and choose what suits you.

How?

Referrals come from a variety of sources.

Following receipt of referral, a member of the team will make contact to obtain background information and agree a contract for the service to be provided to ensure your expectations are met.

Case review meetings, use of outcome measures, SMART and client centred goals, reports and evaluations provide feedback to interested parties. An interdisciplinary approach promotes effective communication, holistic and shared goals that maximise the benefit of services.

Close working with Case Managers, Solicitors, NHS Colleagues, care staff, voluntary organisations and others working with the client provides an integrated service.
